2022 Client Holiday Application

UCAN CANNOT GUARANTEE YOU WILL BE MATCHED WITH A SPONSOR. UCAN RESERVES THE RIGHT TO REFUSE SERVICE.

CAN I APPLY FOR THESE PROGRAMS IF I PLAN TO PARTICIPATE IN SHOP WITH A COP, ANGEL TREE, SALVATION ARMY, ETC? 
No.

If you sign up for more than one holiday program, we will not approve your application. If you are invited to sign up for a program through your school, please let them know you are already working with UCAN.  The only ADDITIONAL program you can sign up for and still qualify for our program is Toys for Tots. Families with younger children, are encouraged to reach out to Toys for Tots to select a special item for your children age 12 and under.

HOW DO I SIGN UP?
We are updating to a new computer system, so every family must complete our NEW CLIENT application including income verifications in order to qualify for our holiday programs. If you need help filling out the forms, please contact  (515) 334-2684. New Client application HERE.

HOW DO I QUALIFY?
If you are a resident of Urbandale and you meet the income guidelines you can sign up. If we have additional resources and/or your children are students of the Urbandale School District you will be able to register starting Nov. 16. If you have never accessed UCAN’s services, please fill out a NEW CLIENT intake form.

HOW WILL I RECEIVE MY ITEMS?
You will receive an email, text, or phone call with pick up instructions, including date and time. Your donor may choose to delivery directly to you.

PLEASE NOTE: If you do not pick up your items or make other arrangements for pick up, your items will be given to families on the waiting list. Failure to pick up may disqualify your family from future programs.

If you do not upload the information your application will be deleted. If you need assistance, please call our office at 515-334-2684 to schedule a time to bring your documents in. Please keep in mind, we will approve all applications as they are received by our office.
